Patient Resources

St. Joseph’s Continuing Care Centre, Sudbury, is a 64-bed continuing care hospital. It is built on South Bay Road, on a beautiful property overlooking Lake Ramsey. St. Joseph’s Continuing Care Centre will continue the legacy of Excellence in Aging and Wellness that was started by St. Joseph’s Health Centre in 2003.

Guided by our core values of dignity, excellence, service and integrity, our approach to holistic care respects the unique needs and fosters the spiritual, physical, and emotional well-being of our patients, staff and volunteers. These values are reflected in the senior-friendly design of our building and the manner in which our staff and volunteers care for our patients.

St. Joseph’s Continuing Care Centre contributes to an expanded continuum of care for the Sudbury area and will nurture the body, mind, and spirit of all patients at various stages of their life journey. Our focus is on providing rehabilitative and supportive care to our patients, with the goal of encouraging independence in activities of daily living and transitioning them to back to their home or a lower, more appropriate level of care. Our philosophy of care respects the individual needs of the patients while working towards increased independence.

St. Joseph’s Continuing Care Centre strives for excellence by continuing the tradition of “Caring Beyond the Moment.” We wish you well as you work to achieve your rehabilitative goals in the weeks ahead.

Spiritual / Religious Care

The Spiritual and Religious Care department of St. Joseph’s Continuing Care Centre recognizes the need for emotional, transitional, and at times grief support. In order to assist our patients, we offer one on one pastoral visits (for both social and spiritual support), Roman Catholic Mass every Sunday at 2 p.m., Communion every Thursday and three special services throughout the year at Easter, Christmas and Thanksgiving.

We also offer the Guardian Angel program for patients who become palliative during their stay at SJCCC. This program is a unique program designed to assist the patient and their families during the end stages of palliation.
