St. Joseph’s Continuing Care Centre (SJCCC) is a 64-bed class E and class G hospital that opened on June 1, 2009, as the first stand-alone continuing care facility in the North East Local Health Integration Network (NE LHIN). St. Joseph’s Continuing Care Centre contributes to an expanded continuum of care for the Sudbury area and will nurture the body, mind, and spirit of all patients at various stages of their life journey.
St. Joseph’s Continuing Care Centre of Sudbury was provincially incorporated and received its letter patent as a non-profit corporation on January 18, 2008, and a Registered Charity on September 22, 2009.
On April 6, 2020, in response to a community need for additional capacity to respond to the threat of COVID-19, 30 additional beds were opened at an alternate location as an expansion of the programming already provided by St. Joseph’s Continuing Care Centre. Further expansion occurred on October 5th, bringing the total number of beds in the Elm street location to 60.
Within our programs at both locations (South Bay Road and Elm Street), we provide specialized care and services to adults requiring rehabilitative, supportive, or transitional care to maximize their functional potential. The goal is to work with these patients to transition them back to independent living, whether that be home, a retirement residence or an assisted living environment.
Our focus is on rehabilitative and restorative care which encompasses participation in daily functional activities which are individualized to meet each patient’s unique needs. This can include participation in a daily exercise program and use of the therapeutic gym; therapy which focuses on restoring independence for basic care needs such as dressing and grooming; use of therapy amenities (i.e., stairs, kitchen, laundry) to enable independence with higher level tasks. Patients will receive customized discharge planning for the opportunity for a successful community reintegration.
